Shining StarShining Star Recognition Award


Purpose:

1. To demonstrate appreciation for contributions and support of our people from which a CHA/P has positively impacted a patient, a community or cause and to recognize those who make sacrifices to serve their communities and patients. In many communities, CHA/Ps remain the single providers who persevere through long hours, hardships, and isolation.

2. To garner statewide recognition and support for these dedicated and brave healthcare providers - the Community Health Aides and Community Health Practitioners.

Criteria:

1. A certified CHA/P who is currently employed by a regional health organization or tribe serving his/her community and is in good standing with the organization.

2. A nominee may be recommended by a co-worker, CI/SI, instructor, or the employer of CHA/P. Nominations are also welcome from patients, the community or board members.

Categories:

1. New motivated CHA - Recipient will receive a trophy for excellent service to their community. Specifically awarded to a new CHA.
2. Experienced CHA/P - Recipient will receive a trophy for their hard work and devotion. The criteria is a minimum of five consecutive years served.
3. In Memoriam - This award is only given out if a deceased CHA/P has been nominated. This award is to memorialize a CHA/P who gave outstanding service and patient care to their community.
4. Outstanding Accomplishments in the Community - Recipient will receive a certificate for accomplishments in the their scope of work. For example - 100% immunizations, preventative health care, emergency care, etc.
5. 100% PEFs - Recipient will receive a certificate for submitting 100% of their PEFs during a minimum of a 3-month timeframe.
